No Man's Sky: Find out portal address and take

To portal travel in No Man's Sky you should know this info:

• You can theoretically travel from any portal to any other portals.
• But for this you need the exact address. The input is done with glyphs. You must have collected all 16 glyphs to be able to enter all addresses. This is done, for example, by completing the story.
• Each portal address consists of twelve glyphs. The address of the portal you are at, you get through interaction - there is namely a query available to you in the dialog options. It is worth saving the address via screenshot.
